Amended Acute Resuscitation Plan (ARP) for adults is now available


An amended Acute Resuscitation Plan (ARP) form is now available for use in any health setting (nursing homes, community residential care, outpatient clinics, GP practices or the patient’s home) in Queensland.
Amendments to the ARP form include:
  • A disclaimer added to the beginning of the form highlighting Queensland Health exclude all warranties, representations and liability in relation to the use of the ARP form by non-Queensland Health facilities; and use of the ARP form by non-Queensland Health facilities; and use of the form by non-Queensland Health services and facilities is subject to that service’s policies and procedures.
  • A patient label sticker on page two.
  • Minor edits for clarity and brevity in the ARP Quick Guide.
  • In Section 6 – clinician authorisation:
    • Changes to the duration an ARP Form can be active to:
      • For this admission/attendance
      • Until date (not longer than 12 months)
      • For 12 months
    • Addition of practice/facility name, role and qualification.
You can access the new ARP form, ARP cover sheet, fact sheets, how to order and other relevant resources here.
ARPs are not currently processed by the Office of Advance Care Planning. Active and voided ARP forms should be filed according to your local service/facility processes.
